The cargo plane crash in Miami is raising concerns about the safety culture at 21 Air, one of the carriers that fly for Amazon. Former pilots were sounding alarms even before Sunday's accident.

This comes after the widow of one of the five victims filed a wrongful death lawsuit on Wednesday that named Amazon, the airplane's operator, 21 Air, and the two pilots, Joseph Caroll and Jaime Felipe Silva Moreno, as defendants.

Former employees alleged that 21 Air previously ignored protocol and suppressed safety reports. The company said it has been deploying the right training and procedures.

Crews started removing parts of the plane overnight while a cockpit transcript reveals pilots knew speed was excessive.
